The intima does a superb job of protecting the artery from clogging for many decades.

However, after years of exposure to factors that promote development of atherosclerosis, (high blood pressure, diabetes, high cholesterol, smoking) this barrier breaks down and the process begins.

There is a progressive inflammation

(swelling as a result of invasion by white blood cells) and deposition of lipid (fatty) material and calcium into the sub-intimal space.

 This build up of material is commonly called plaque formation.
